تبدأ أحداث هذا اللغز بتعرف المغامرين الخمسة على الأستاذ قاسم الكويتي عندما دلوه على الشارع الذي تسكنه أخته وهناك تفاجأ المغامرون بسرقة نقود شاكر الرجل الذي يسكن في المنزل المقابل لمنزل أخت الأستاذ قاسم ووجد المغامرون الخمسة أنفسهم أمام ثلاثة ألغاز في لغز واحد معقد جدًا.

يأتي رجلا يدعى بيرسي تريفيليان، طبيب عام، إلى شرلوك هولمز لاستشارته في قضية غريبة.يقول تريفيليان انه كان طالبا ذكيا لكن لم يكن يملك المال الكافي، إلا أن رجلا يدعى بليسنغتون عرض عليه المساعدة، حيث ساعده في فتح عيادة ودفع جميع نفقاته، وفي المقابل اشترط على تريفيليان أن يعطيه ثلاثة أرباع ما يجنيه في اليوم وأن يعيش معه.

الآنسة سوزان كوشينغ من كوريدون(بلدة في جنوب لندن) تستقبل طردا عن طريق البريد يحتوي على اذنين بشريتين محفوظتان بواسطة ملح خشن. المفتشة ليستراد من سكوتلاند يارد تشك في أن الأمر عبارة عن مزحة من ثلاث طلاب يدرسون الطب قامت الآنسة كوشينغ بإجبارهم على مغادرة السكن، وما يزيذ من اقتناعه بفرضيته أن الطرد كان مرسلا من بلفاست، مسقط رأس أحد الطلاب. بعد أن يقوم هولمز بفحص الطرد بنفسه يقتنع أن له علاقة بجريمة خطيرة وليس مجرد مزحة من الطلاب الثلاث، لأنه لو كان فعلا طالب طب هو من قام بإرساله لكان استعمل شيئا آخر لحفظ الأذنين بدل الملح.
